- signed visa application form; - applicant’s valid passport/travel document; - 1 applicant’s photo (35 * 45 mm); - certificate of a person of Ukrainian descent issued by Ukraine’s National Committee on persons of Ukrainian descent or applicant’s statement on his Ukrainian origin along with supporting documents.
