Visa Application Forms of minors shall be submitted with signature of a parent or legal guardian - One Color passport size photograph – maximum 6 months old 35 mm x 45 mm - Original valid passport that is valid for at least three months after the stated date of departure from Ukraine; have at least two blank pages and has a validity period of no longer than 10 years - Valid Travel Health Insurance with coverage of 30,000 EUR covering emergency medical expenses, hospital treatment, repatriation for medical reasons or death. It must be valid in the whole Schengen area or worldwide Insurance should cover the period of stay and an extra 5 days in original plus copy. - Proof of applicant’s financial sufficiency for the intended period of stay and return to the country or origin. An account statement has to be issued for the last 3 months, with an approximate settlement of USD 45 x(number of days of stay + 5 extra days) in original form 3- month Current Bank Statement, stamped and signed by the Bank. If using company Statements certificates of Registration documents showing ownership. - Payment slips for consular fee, unless otherwise stipulated.
