Visa application form: filled out and signed by the applicant, or a parent or legal guardian on behalf of minor (under the age of 18). Travel document: issued in the last 10 years, 2 empty pages, expiry date exceeding requested visa by 3 months. Face photograph: in colour, 35x45 mm; with neutral facial expression and shown person's cheeks, chin and forehead, if the person is wearing scarf for religious or medical reasons. Travel medical insurance: valid for Schengen area, covering any expenses which might arise in connection with repatriation for medical reasons, urgency medical attention, emergency hospital treatment or death during the stay. The minimum coverage € 30.000. Means of transportation: return plane ticket; If the person is travelling to a third country, proof of itinerary and visa, if necessary. Accommodation: booking hotel, private or other accommodation, real estate title deed or lease, etc. Accommodation can be provided by the guarantor or hosting party - it shell be stated in the letter of guarantee or invitation or in a written request. Social & professional status: certificate of employment, school attestation, proof of registered commerce, etc. Financial means: pay or pension slips and/or bank account statements for the last 3 months, proof of scholarship, etc. Persons without their own resources: statement on sponsorship with proof of the sponsors’ financial funds. Funds must be equivalent to EUR 70 per day of the visit. If the visitor has submitted a proof of ensured accommodation, he/she must possess amount equivalent of EUR 30 per day of the visit. Exception: if stated in the letter of guarantee or written request, that host or guarantor is covering all expenses. Proof of the purpose of stay in Croatia: - Tourism: proof of accommodation, itinerary - Private or business visit: original Letter of Guarantee (filled and with certified signature for private visit, or signed by authorized person for business visits) with proof of solvency of the guarantor; - Members of official delegations: letter of competent state authority confirming that the applicant is a member of its delegation and a Letter of (official) Invitation - Attending a conference: original Letter of Guarantee (signed by authorized person) with proof of solvency of the guarantor except when the event is under the patronage of Croatian Parliament, President of the Republic of Croatia or the Government of the Republic of Croatia or if it is an international event of public interest, in which case instead of the guarantee letter, an invitation letter has to be delivered containing the following information: - Information about the applicant – name, surname, date and place of birth, citizenship, passport number, place of issue and validity of passport - Purpose of the visit - Period in which the applicant will stay in Croatia - Information regarding accommodation - Information regarding who will bear the costs of the visit to Croatia - Name, surname, position, address and phone number of the contact person in the government authority or in the organizer of the event, that can provide relevant information about the applicant and his/her visit to Croatia Members of the immediate family of a citizen of Croatia/EU/EEA/Switzerland: proof of the family tie (birth or marriage certificate (with apostille or fully legalized depending on where the marriage took place) with photocopy of the ID card/passport of the Croatian/EU/EEA/Swiss citizen;), proof they are traveling together to Croatia or that they are joining their Croatia/EU/EEA/Switzerland family member in Croatia (exonerated from proof of financial means & social and professional status & travel insurance & accommodation). For minors: Birth certificate (with apostille or fully legalized depending on where the child was born) if applicable: the decision of the competent authority on appointing the legal guardian. Minors travelling alone or without parent or legal guardian: original authorization (with certified signature and apostilled or fully legalized) of a legal guardian or parent, containing personal information of the legal guardian, purpose, place and length of stay in Croatia, as well as the period for which the authorization is issued and details regarding the person in charge of the minor during the visit to Croatia.
