- original of receipt of payment for VISA FEE in USD made to bank account of embassy in Tehran + COPY. - signed visa application form filled (visa.mfa.gov.ua ↗) with 1 applicant’s photo (35 * 45 mm) glued to upper left of first page of application form; - applicant’s valid passport/travel document with signature + COPY; - copy of all pages + original birth certificate - residence documents – valid residence permits/equivalents (in cases where applicants reside in a country that is different from the country of their nationality/citizenship); - proof that the applicant is a holder of (or is entitled to benefit from) a valid medical/travel insurance policy to cover Ukraine and the period of stay (the policy should provide a minimum of €30 000 or equivalent coverage);+ COPY - the original of official bank letter confirming that there is an adequate financial security, as per embassy the required amount is USD 45 per day. If the amount is specified in Rial, letter should include exchange rate to USD or EUR. - invitation letter from an entity/company/ organization registered in Ukraine in official notary, presented on the official letterhead, with indication of the entity/company/ organization’s number according to Ukraine’s Unified State Registry of Legal Entities, Individual Entrepreneurs and Citizens’ Associations, registration number, date and signature, full name of the invited person, information about his/her date and place of birth, nationality, passport details, place of residence, purpose of visit, duration of intended visit to Ukraine, number of entries and place of stay in Ukraine, legal person’s obligations towards possible costs related to the invited person’s stay and/or departure from Ukraine; + COPY - Original letter issued by an Iranian company or employer concerning the job position, the duration of working relationship, etc. , as well as the purpose of the trip - copy of (signed) passport of manager or the person who has the authority to sign the invitation. - Gazette of inviting company or organization. - proof of address of accommodation of applicant. (Hotel booking, resort, etc.) - proof that the applicant is a holder of (or is entitled to benefit from) a valid medical/travel insurance policy to cover Ukraine and the period of stay (the policy should provide a minimum of €30 000 or equivalent coverage);+ COPY - round flight ticket (reservation will be enough). In the usual business invitations, the accommodation of the hotel or similar accommodations is mentioned if invited. The following documents are required for a private home or apartment:(A copy of the title deed or a document that specifies the status of its ownership, such as власності право про свідоцтво, ВИТЯГ,..... or технический паспорт - If the owner of the property and the invitee are different: Copy of the owner's passport + manual consent from the owner with the name and information of the guest and the duration of the trip - If the owner and the inviter do not have a relationship with each other, it is better for the property owner to send an invitation letter to the applicant.
