Please copy all previous (valid and expired) visas and used pages. - Face photograph: in colour, 35x45 mm, printed on normal photographic paper, taken against a light background; full face taken straight forward 30-36 mm from point of the chin to the head of hair; the person is to be captured with neutral facial expression, with his/her eyes open and mouth closed. The photograph of a person, who for religious or medical reasons is wearing a head cover, must show the person's cheeks, chin and forehead. - Travel health insurance to cover the possible expenses related to repatriation, for health reasons, urgent medical assistance and/or urgent hospitalization, with the minimum coverage of EUR 30 000. The insurance must be valid for the territory of the Republic of Croatia during the entire stay in Croatia. - Proof of residence for nationals other than the country of application- a photocopy of the stay permit. - Cover letter: explaining the purpose of the trip and duration of stay. - Means of transportation and intention to return to the country of residence: return travel ticket for airplane, bus, or boat; registration card and driver's license if travelling by car; If the person is travelling to a third country, then proof of itinerary (travel ticket, etc.). - Proof of the purpose of stay in Croatia: Original Letter of guarantee filled out by a guarantor β legal person from Croatia (format Jamstveno pismo pravne osobe**) (processing can start with the scan copy of the Letter but for issuance of visa, if approved, original of the same has to be submitted). Guarantor β legal person must enclose to the Letter of guarantee a document proving its liquidity (βBon 1β, βBon 2β, certificate of the commercial court that no liquidation proceedings have been initiated, etc.).
