- For Iraqi Passport holder: Step 1 - Completed application form (please complete all parts as clearly as possible). - One (1) color photos of applicant (size 3.5 x 4.5 cm.) - Copy of National ID and valid Passport (must be valid for at least 6 months from your intended arrival into the Kingdom) together in 1 A4-sized paper. Step 2 After approval - Original Passport (must be valid for at least 6 months from your intended arrival into the Kingdom). - 3 Completed application form with renewed proposed date of arrival. - Three (3) color photos of applicant (size 3.5 x 4.5 cm.) - Copy of an Airline ticket or confirmation slip with applicant's name, flight number as well as date of entry and departure. - Copy of an accommodation bookings for the entire stay in Thailand. - For Jordan Passport holders: - Original Passport - Completed application form (please complete all parts as clearly as possible). - Two (2) colour photos of applicant (size 3.5 x 4.5 cm.) - Copy of valid Passport (must be valid for at least 6 months from the intended arrival into the Kingdom). - Copy of National ID.
