The agency shall submit the following documents related to a specific travel: - a certificate of travel package - a list of travelers (name, surname, passport number) - information about accommodation in Croatia (address) - information about transportation - group or individual travel health insurance - a proof of paid visa fees and for each traveler: - a filled out visa application form, signed manu propria - a valid passport - a 35x45 mm color photo - Visa application form: to be filled out and signed by the customer , or a parent or legal guardian on behalf of minors: mvep.hr β (mvep.hr β) The application form can also be filled out online at crovisa.mvep.hr β. (crovisa.mvep.hr β) - Passport: issued within the last 10 years, with expiry date exceeding the expiry date of the requested visa by a minimum of three months, and containing at least two empty pages. Please copy all previous (valid and expired) visas and used pages. - One photograph: please see the information on the Photo Specification page. - Travel health insurance to cover the possible expenses related to repatriation, for health reasons, urgent medical assistance and/or urgent hospitalization, with the minimum coverage of EUR 30 000. The insurance must be valid for the territory of the Republic of Croatia during the entire stay in Croatia. The customer for a multiple-entry visa must have travel health-insurance policy that covers the period of the first visit to Croatia.
