This list is intended to help you gather documents for obtaining a visa. Please note that the Embassy/Consulate may ask you to provide additional documents or additional information if necessary to make a decision on your application. Documents to be prepared by the applicant: - Visa Application form in 2 copies in English or Japanese with applicant’s signature and date (parent’s signature is required in child’s application) - 2 photos (3.5 x 4.5) - Foreign passport - Copy of Foreign passport (page with photo only) - Internal passport with registration (copies must also be attached to the application). Foreign citizens must provide documents confirming legal, long-term stay in Russia - A certificate from a medical institution on planned medical actions and a letter of guarantee from the host guarantor organization. - Information about the medical institution and the guarantor: it is necessary to indicate the name of the medical institution or organization, address, registration number, surname and name of the inviting person, telephone and fax numbers, as well as affix the seal of the organization. - If there are several accompanying persons, make a list of applicants. - Information about the patient and the accompanying person: surname and first name must be indicated in Latin letters in accordance with the spelling in the passport. - Financial document (e.g. bank statement) - In case of multiple entries and/or stay in Japan for more than 90 days: - Certificate of Status of Stay in Japan (if the period of stay in Japan exceeds 90 days for the purpose of hospitalization) - Treatment plan (only if it is necessary to enter Japan multiple times for the purpose of treatment). The document is provided by the host organization. The stay plan should be described in as much detail as possible, indicating the date of entry into Japan and the date of departure from Japan. Documents to be prepared by the applicant: - International passport - Visa Application Form (https://www.mofa.go.jp/mofaj/files/000124525.pdf) in 2 copies in English or Japanese with personal applicant’s signature and date (one of the parents signs for the child) - 2 photos (35 x 45 mm) - Copies of the Internal Passport (front page + all pages with registration stamps); if the last stamp is a deregistration stamp, then copy of the next blank page must be attached - Documents confirming employment (type of occupation)(not older than 1 month): - All applicants must provide the original certificate from Russian bank in Russian or English with a live seal and signature of a bank employee (printed certificates from a personal account or online statement are not accepted). - For employed: the original certificate of employment with a live seal and the signature of the employee indicating the position and salary (in Russian or English). - For individual entrepreneurs: a copy of the certificate of registration of the individual entrepreneur (if received before 2017), a copy of the certificate of registration with the tax authority and an extract from the Unified State Register of Individual Entrepreneurs. - For the self-employed: a copy of certificate of registration with the tax authority and the status of calculations (income) for professional income tax (in the form of KND 1122035 and 1122036). - For minors: a sponsorship letter from parent (in Russian or English), a copy of the document confirming the relationship with the sponsor, the original certificate from the sponsor's work indicating the position and salary, as well as the original bank certificate in Russian or English about the balance of the sponsor's funds (when applying together, copies of work certificate and bank certificate can be attached). - For students: original sealed certificate from the place of study. - For the unemployed: the original certificate from the branch of a Russian bank in Russian or English on the balance of funds with a "wet" seal and the signature of the bank employee. - For pensioners: a copy of the pension certificate. - Confirmation of flight reservations (in Russian or English). In case of a group submission, must be attached to each application. - The Schedule of Stay in Japan (in English or Japanese). The date of entry into Japan and the date of departure from Japan must be indicated, as well as the flight number, hotels’ names, addresses, telephone numbers, and detailed information about places which will be visited each day. In case of group submission must be attached to each application with specified surname and the number of accompanying persons. - A letter in English or Japanese explaining the need for multiple visits (In case of applying for a multiple-entry visa).
