- signed visa application form; - applicant’s valid passport/travel document; - 1 applicant’s photo (35 * 45 mm); - certificate of a person of Ukrainian descent issued by Ukraine’s National Committee on persons of Ukrainian descent or applicant’s statement on his Ukrainian origin along with supporting documents. If applicant’s family applies for this type of visa as well documents on their status and family relation to the applicant should be provided; - proof of medical/travel insurance to cover Ukraine and the period of stay (the policy should provide 30 000 euros or equivalent minimum coverage); All applicants, that apply through visa centers, should submit an insurance from Ukrainian insurance company that covers the treatment/observation and other issues related to COVID-19 (this should be written in the insurance document).
