- This list is intended to help you compile the documents for your visa application. Please note that the Embassy may ask you for additional documents or further information if this is necessary to assess your application. - Visa application form: - Visa application form, filled in large block letters in Croatian or English language, personally signed by the applicant in the following points: - 3rd page clause 37 - Signature (for minors: the signature of both parents is required, if only one parent signs - the second must provide notarized consent for the submission of his / her child's documents) - 4th page - Signature (for minors: the signature of both parents is required if only one parent signs - the second must provide a notarized consent to the submission of his / her child's documents) - (on-line form: , crovisa.mvep.hr ↗ (crovisa.mvep.hr ↗) ) - If you can’t prepare the Visa Application form by yourself, our manager can fill the Visa application form for you - Travel insurance is mandatory for Croatia. - Travel document: passport issued in the last 10 years, 2 empty pages, expiry date exceeding requested visa by 3 months. - Face photograph: in color, 35x45 mm; with neutral facial expression and shown person's cheeks, chin and forehead, if the person is wearing scarf for religious or medical reasons, not older than 6 months - For minors: - Both parents must be present when submitting documents. If only one parent is submitting documents, he / she must provide notarial consent from the other for the submission of documents for a visa to Croatia for his / her child. - For children, it is always necessary to fill out a separate visa application form, even if they are endorsed in the parents' passport. - For minors (under 18 years of age), the application form must be signed by both parents or by the person exercising temporary or permanent parental rights or legal guardianship. - Birth certificate; - if applicable: the decision of the competent authority on appointing the legal guardian, death certificate of deceased parent. - Minors travelling alone or without one parent or legal guardian: an original authorization (certified signature by Notary public) of the legal guardian or parent(s), containing personal information of the legal guardian, child and the person the child is traveling with, purpose, place and length of stay in Croatia, as well as the period for which the authorization is issued. - If the child is traveling with parents who already have a valid Croatian or other visa enabling them to enter and stay in Croatia, a copy of the said visa of the parent(s) travelling with the minor for whom a visa is not applied at the same time with the parent(s) and the reservation of airplane tickets for the parents for the same time period. - Travel health insurance: valid for the territory of Croatia, covering any expenses which might arise in connection with repatriation for medical reasons, urgency medical attention, emergency hospital treatment or repatriation of mortal remains. The minimum coverage EUR 30.000.+ Covid 19 - Means of transport and the intention to return to the country of origin or to a third country: - return plane/train/bus/boat ticket; - driver license and auto-registration certificate if travelling by car; - If the person is travelling to a third country, proof of itinerary and visa (if necessary). - Certificate of employment, school attestation, proof of registered commerce, etc. - Accommodation: booking hotel, private or other accommodation, real estate title deed or lease, etc. Accommodation can be provided by the guarantor or hosting party - it shall be stated as such in the letter of guarantee or invitation or in a written request. - You can print or copy all required documents in our visa application center - Financial means - This requirement may be waived if in the letter of guarantee or written request stated that host or guarantor is covering all expenses. This requirement is waived for members of official delegations. [Table:Official Visit ] - Copy of the internal passport (issued at the age of 14): pages concerning the applicant's biodata, foreign passport(s) issued to him/her, his/her marital status and registration in Russia. - You can print or copy all required documents in our visa application center - In case of third-country nationals, proof of legal residence in the Russian Federation in line with federal legislation (e.g. residence permit, long-term visa ), valid for at least three months after the planned return from Croatia or a document proving that the applicant has applied for a renewal of proof of legal residence - I. For non-Russian nationals - For members of official delegations who, following an official invitation, shall participate in meetings, consultations, negotiations or exchange programmes, as well as in events held by intergovernmental organizations: - letter issued by a competent Russian authority or a verbal note from the MID confirming that the applicant is a member of its delegation travelling to Croatia - copy of the official invitation from the relevant Croatian body/intergovernmental organisation inviting the delegation on the official visit to Croatia containing the following information: - for the invited person – name and surname, date of birth, sex, citizenship, number of the identity document, time and purpose of the journey, number of entries; - for the inviting legal person, company or organization - full name and address, the name and position of the person who signs the request, and if the inviting person is a legal person or company or an office or their branch established in Croatia – Tax Number (PIN) - Participants in official twin city programmes: - written request from the head of administration/mayor of the city where the twinning activity is to take place. - The written request shall contain the following data: - for the invited person – name and surname, date of birth, sex, citizenship, number of the identity document, time and purpose of the journey, number of entries; - for the inviting legal person, company or organization - full name and address, the name and position of the person who signs the request, and if the inviting person is a legal person or company or an office or their branch established in Croatia – Tax Number (PIN) - Written request from the head of administration/mayor of the sending city with full details of the participant. - II. For non-Tajik nationals - For members of official delegations - letter (e.g. verbal note) issued by a competent authority confirming that the applicant is a member of its delegation travelling to Croatia - copy of the official invitation from the relevant Croatian authority inviting the delegation on the official visit to Croatia containing the following information: - for the invited person – name and surname, date of birth, sex, citizenship, number of the identity document, time and purpose of the journey, number of entries; - for the inviting legal person, company or organization - full name and address, the name and position of the person who signs the request, and if the inviting person is a legal person or company or an office or their branch established in Croatia – Tax Number (PIN)

The purpose of stay and accommodation is proven by: - hotel, hostel, camp … - confirmation of hotel or other accommodation booking with names and surnames of all customers, who will apply for visa, proof of payment for package holiday, organized tourist travel, etc. - private accommodation – confirmation of reservation, along with a copy of a competent office’s decision authorizing renting services (local government decision/trade license) - use of (personal) property – proof of property ownership (title deed/land register certificate), lease agreement; invitation/declaration by an owner who is a foreign national - nautical tourism – confirmation of berth reservation/payment in Croatia, excerpt from the register of yachts, charter payment - one-day trips to Croatia (from Montenegro, Bosnia and Herzegovina) – travel agency’s payment confirmation; rent-a-car confirmation, etc. Accredited travel agency The application shall be submitted by an authorized representative of a travel agency for individual or group travels. The agency shall submit the following documents related to a specific travel: - a certificate of travel package - a list of travelers (name, surname, passport number) - information about accommodation in Croatia (address) - information about transportation - group or individual travel health insurance - a proof of paid visa fees and for each traveler: - a filled out visa application form, signed manu propria - a valid passport - a 35x45 mm color photo The Embassy reserves the right to request additional documents or interview any person from a group of travelers./ The customers are requested to read the basic requirements carefully and conform to the same while submitting the application. - Visa application form: to be filled out and signed by the customer , or a parent or legal guardian on behalf of minors: The application form can also be filled out online at crovisa.mvep.hr ↗. Please copy all previous (valid and expired) visas and used pages. - One photograph: please see the information on the Photo Specification page. - Travel health insurance to cover the possible expenses related to repatriation, for health reasons, urgent medical assistance and/or urgent hospitalization, with the minimum coverage of EUR 30 000. The insurance must be valid for the territory of the Republic of Croatia during the entire stay in Croatia. The customer for a multiple-entry visa must have travel health-insurance policy that covers the period of the first visit to Croatia. Notice: travel health-insurance should be passed in original - Proof of residence for nationals other than Tajikistan: a photocopy of the stay permit; - Means of transportation and intention to return to the country of residence: reservation of return travel ticket for airplane, bus, or boat; registration card and driver's license if travelling by car; if the person is travelling to a third country, then proof of itinerary (travel ticket, etc.). - Proof of the purpose of stay in Croatia: - certificate of a paid tourist package; - booking of a hotel or other accommodation; - private accommodation booking (copy of the decision of the competent office in Croatia approving the renting) - day to day itinerary with ensured accommodation for the entire stay - proof of ownership or lease of a boat/yacht etc.; - Documents proving sufficient funds to support the stay in Croatia and to return to the state of residence or to travel to a third state: - 70 EUR per day of the estimated stay in Croatia - 30 EUR per day of the estimated stay in Croatia if a third-country national is in possession proof of paid tourist package, or similar document - The supporting documents may include: pay slip and/or bank account statements about the balance and transactions on your bank account in the last three months, a pension slip, proof of scholarship received etc. - Proof of professional or student status: evidence of employment, certificate of school attendance or proof of studying etc.; for retired persons – proof of pension; - For minors and persons deprived of legal capacity: visa application shall be submitted by a parent/legal representative, including: - birth certificate of the minor (if parents are legal guardians) - decision of the competent body designating the legal guardian - If a minor is traveling without a parent/legal guardian, (s)he must carry a notarized consent of a legal guardian that shall contain personal information of the parent/legal guardian and the purpose, the period and length of stay in Croatia, the period for which the consent is issued, and the signature of the parent/legal guardian. - The documents attached shall not be older than three months. - Croatian Embassy may require you to submit additional supporting documents and/or certified translation as well as the international certification (apostille) of the documents during the visa application process.

Notice: travel health-insurance should be passed in original You can also opt for Insurance service during the submission of your documents. - Proof of residence for nationals other than Kyrgyzstan: a photocopy of the stay permit; - Means of transportation and intention to return to the country of residence: reservation of return travel ticket for airplane, bus, or boat; registration card and driver's license if travelling by car; If the person is travelling to a third country, then proof of itinerary (travel ticket, etc.). - Proof of the purpose of stay in Croatia: A Letter of guarantee by natural person shall be submitted in original or copy if the original is deposited in the Ministry of Foreign and European Affairs in Zagreb – link PDF checklist ↗ (PDF checklist ↗) For spouses, children, and parents of Croatian nationals: marriage certificate or birth certificate shall be submitted. - For the members of the immediate family of a Croatian citizen and a citizen of the European Economic Area (EEA): Certificate from the Register of Births or Marriages and a photocopy of the ID card of the Croatian citizen or the EEA citizen; - Documents proving sufficient funds to support the stay in Croatia and to return to the state of residence or to travel to a third state: - 70 EUR per day of the estimated stay in Croatia - 30 EUR per day of the estimated stay in Croatia if a third-country national is in possession of a certified Letter of Guarantee by Natural or Legal Person from the Republic of Croatia - Exceptionally, if the Letter of Guarantee indicates the guarantor is fully accepting all the expenses related to visiting and departing from Croatia, the customer can be exempt from the requirement to present the proof of sufficient funds. - The supporting documents may include: pay slip and/or bank account statements about the balance and transactions on your bank account in the last three months, a pension slip, proof of scholarship received etc. - Proof of professional or student status: evidence of employment, certificate of school attendance or proof of studying etc; ; for retired persons – proof of pension - Accommodation: hotel booking, private or other accommodation, real estate title deed or lease, proof of ownership or lease of a boat etc. Accommodation can be provided by the guarantor or the inviting party if so indicated in the Letter of Guarantee or the Letter of Invitation. - For minors: birth certificate, If a minor is traveling without a parent/legal guardian, (s)he must carry a notarized consent of a parent/legal guardian that shall contain personal information of the legal guardian and the purpose, the period and length of stay in Croatia, the period for which the consent is issued, and the signature of the parent/legal guardian.
