Temporary permit must be valid for at least 3 months after the planned return from the Schengen Area. - Travel health insurance to cover possible expenses for medical assistance, hospitalization and repatriation, with the minimum coverage of EUR 30 000 valid for all Schengen Area Member States, for the whole duration of stay. - Means of transportation: reservation of return air travel ticket with dates and flight numbers specifying entry and exit from the Schengen Area. Modes of transport in or between Schengen States (boat, train, car, bus). If traveling by car, car rental agreement and driver's license. - Proof of the purpose of stay in Croatia: TOURISM: proof of a paid tourist package; hotel booking or other (private) accommodation booking; precise day to day itinerary; proof of ownership or lease of a boat/yacht, paid excursions etc. SPORT / CULTURAL ACTIVITY: Letter of Guarantee by Legal Person or Letter of Guarantee by Natural Person: in original or a photocopy if the original has been deposited at the Ministry of Foreign and European Affairs in Croatia. Letter of invitation issued by a Croatian authority, or the organizer of an international meeting or manifestation in Croatia. - Proof of professional or student status: signed and stamped evidence of employment stating position, length of service, salary. If self-employed: business registration documentation and/or TIN certificate. Students: signed and stamped letter from school/ university certifying the student status of the applicant. Retirees: proof of pension. - Proof of sufficient funds stamped bank statement (last 3 months, in original). 70 EUR (or TZS equivalent) per day of the estimated stay in Croatia; 30 EUR per day of the estimated stay in Croatia if a third-country national has paid an organized tourist package (accommodation and transportation included in the package). - For minors and persons deprived of legal capacity: application to be submitted by a parent / legal representative. For minors the birth certificate and both parents’ certified copies of IDs or passports must be attached. If the applicant intends to enter the territory of a Schengen Member State, however, short the stay will be (including change of airports, going from an airport to a train or bus station, or even change of terminals when that entails entering the territory of the Member State), he/she must apply for a short stay visa (Type-C). Nationals of countries which are required to have a Transit Visa / Airport Transit Visa Type-A must present: flight ticket, proof of entry permit (visa) for the country of final destination. Any document attached shall not be older than three months. List of supporting documents to be submitted by applicants travelling for medical treatment - A letter from the clinic/host medical institution confirming the applicant’s appointment at the clinic/medical institution in question. - Financial guarantee: proof of a bank payment or transfer of the cost of the treatment to the clinic, on the basis of the payment plan agreed with the clinic. - Medical/health documentation issued by the applicant’s country of residence attesting applicant’s illness; declaration by a public or private healthcare facility (which must be accredited by the National Health Service) indicating the type of treatment, the date it will begin, its estimated duration and the costs associated with it.
