Checklist for Re-entry Visa - Visa application form, filled in completely in block letters, and signed by the applicant. - Signed Passport (valid for at least 3 months longer than intended stay), +1 photo copy of the said passport (without the blank pages) - Previous passports, (if any) +1 photocopy of pages bearing particulars of the applicant - 2 recent passport size photographs (not more than 3 months old) with applicants usual signature at the back - Visa Fee + VFS Charges - In the case of a theft or a loss of residence permit, a copy of the report of loss or theft of the local police force translated into Italian - In the case of the expiry of a residence permit within a time lower than 60 days, the original or the copy of the expired document and a copy of the receipt of request for renewal Clarification on the acceptance of visa applications of minors who are not accompanied by at least one parent.
