For citizens of Ukraine and permanent residents legally residing on the territory of Ukraine who wish to apply for a Culture visa category following documents are required: - Visa Application form - Completed for each applicant including children. Please, pay attention that the certificate of cash flow will not be accepted as proof of solvency. - Travel checks, under the name of the applicant, duly signed, presented in original and copy - Pensioner statement from the Pension Fund of Ukraine indicating the amount of pension gained for the last 6 months. - University certificate that proves the accrual of scholarships for the last 6 months. - Notarized sponsorship letter and a proof of income of the sponsor. - Hotel or other reservation - Hotel reservation or voucher from travel agency needs to contain: personal data of each applicant, date of intended stay, reservation number, hotel address, contact phone number. - For rented private apartments: confirmation of the reservation from the owner (period of stay, names of each applicant, passport numbers) and copy of decision of the Croatian authorities on the legal status of renting business called RJEŠENJE. - Invitation - From host organization - competent authorities to participate in scientific, cultural or artistic activities held in Croatia, containing the following information of the invited person (full name, gender, nationality, date of birth, purpose of travel and / or character events, stay and number of entries). - If the inviting organization covers the costs of travel, meals and accommodation during the stay in Croatia, this fact must be clearly stated in the letter. - Proof of intended transport (at least one must be presented). - Tickets reservation. - Copy of the technical passport of the vehicle, green card and a copy of driver’s license for those who will travel by own vehicle.
