No visa documents are needed for a short visit — Ukrainian citizens enter Croatia with just a valid passport. If you apply for a longer-stay visa, these are the exact documents required, by visa type, from the official visa application centre.

For citizens of Ukraine and permanent residents legally residing on the territory of Ukraine who wish to apply for a Culture visa category following documents are required: - Visa Application form - Completed for each applicant including children. It has to be filled out online. - Please note, visa application form will not be accepted if it is filled in paper form. Application form has to be filled only in the system of Ministry of Foreign Affairs of Croatia and printed. The link to the online application form can be found in the section "Download Form". - Filled out either in Ukrainian language with English transliteration or in English or Croatian languages. The online form needs to be printed after filling and signed personally by the applicant in the field 37 and on the last page.
