The cost of the service is UAH 85. You only need to provide information to the VAC employee who will fill in the data into an electronic form and attach a print-out to the supporting documents. 2. 1 Photograph - In color, 3,5x4,5 cm - Facing forward, white background - Not older than 6 months before the submission date - More detailed information can be found in the <<Photo specifications>> section - You have the opportunity to use the photo booth service in all the Visa Centers of the Embassy of Greece in Ukraine. Directly inside of the Visa Centers there are paid self-service coin operated vending machines. You can see the photographs taken on their screen and choose the best variant. The vending machine is very simple to use, and you can receive ready photos during 45 seconds. The nominal cost of the 4 photos of the passport format (3,5x4,5 cm) is 60 UAH. 3. International passport(s) - The passport must satisfy all criteria enlisted in art 12 of the Visa Code. Must be valid at least 3 months after the planned trip. Passport must contain 2 blank pages marked <<visas>>. - If a child is travelling with a parent's passport, 2 additional pages are required for a visa. Also, please, note that children aged 5 or older must have their photo in the passport. - The customer must also present all other valid passports in original and, if available, the expired ones (not older than 3 years). - Please, note that copies of previous visas can be accepted only if the original is provided. - The customer must submit copies of all the pages of international passport except the blank ones - For those customers who submit their applications in Visa Application Centers under jurisdiction of General Consulate of Greece in Odessa - customers under 18 must provide a separate travel document of a child. 4. Internal passport - The customer must present the original and submit a copy of all pages of the internal passport except the blank ones 5. Travel medical insurance - Travel medical insurance covering the entire period of the intended stay, valid for all Schengen States and covering any expenses which might arise in connection with repatriation for medical reasons, urgency medical attention, emergency hospital treatment or death during the stay. The minimum coverage must be 30.000 EUR in accordance with the art. 15.2 of the Visa Code, for customers receiving visas for multiple entries, the insurance only needs to cover the first trip. - Please, note that you have the opportunity to buy a travel medical insurance directly inside the Visa Application Centre. Please consult the representative of the Insurance Company present at the Visa Application Centre for more details and cost of the travel insurance policy. 6. Minors that are travelling - Written consent from the parent/parents not travelling with the child. The written consent must have a stamp of a Notary public and has to be presented in original only. If there is only one custodian, a birth certificate, the court decision on sole custody, death certificate or other document must be presented to prove this. If the mother is entitled to sole custody in accordance with Art. 135 of the Family Code of Ukraine, you must provide a certificate from the Registry Office that a mother is raising a child alone in accordance with Art. 135 of the Family Code of Ukraine. - For those customers who submit their applications in Visa Application Centers under jurisdiction of General Consulate of Greece in Odessa - if one/both parents have a valid Schengen visa and do not apply for a Greek Schengen visa, it is necessary to provide a written consent where the parent/parents give their permission for a child to travel and apply for a visa. - Copies of all pages of the international and national passport of both parents (except the blank ones). - Work certificates and financial documents of both parents. - Birth certificate - original and copy. 7. Tickets and accommodation - Reservation of tickets and booking of a hotel with the personal details of the travelers. If customers rent a yacht, they must provide: a yacht lease contract, confirmation of payment, a list of all passengers traveling on a cruise, including crew. - If a family is travelling, every member of the family has to present reservations with the list of all travelling family members. 8. Proof of activity - For employed persons: Employment certificate confirming the position of employee, the salary gained within the previous period of 6 months, the date of employment, and the duration of the vacation, - For those customers who submit their applications in Visa Application Centers under jurisdiction of General Consulate of Greece in Odessa - a copy of the work book must be provided. - For private entrepreneurs: - A copy of the certificate of registration of the private company; - Original bank account statement of the private company account for the last 3 months; - Original certificate from the State Tax Administration from the place of registration proving company's payment of the single tax. - For pensioners: Pension certificate, the certificate of amount of the pension gained within the previous period of 6 months. - For pupils: Original certificate from the educational institution, which states that it is issued for submission to the Embassy of Greece. - For students: - Certificate from the university indicating the faculty, curriculum and graduation date, as well as the fact that the certificate is issued to the Embassy of Greece in Ukraine; - A copy of the student card, certified by the university; - A copy of the record book, certified by the university 9. Proof of sufficient financial means - A bank account statement for the last 3 months and a copy of the bank card, - If applicable, the customer may present additional proof of personal means, such as proof of real estate property or proof of car ownership. - Please, note that the minimum amount of funds in the account - 50 euros per day of stay in Greece for one person. Sponsorship - Expenses may be covered by the host or other third persons. The document that proves other person covering such expenses shall be a statement approved by public Notary and proof of income of the sponsor. - Sponsorship letter for the family members (husband, wife, children) may be hand-written or typed, at your discretion. You must also provide proof of family relationship (marriage certificate, birth certificate). For people who are not relatives, (husband, wife or children), sponsorship letter must be notarized. In case of tourism by the means of individual transport - A detailed travel plan. - If the customers are travelling by car - a detailed travel plan, copy of driver license, certificate of car ownership and insurance of the vehicle. - If customers rent a yacht, they must provide: a yacht lease contract, confirmation of payment, a list of all passengers traveling on a cruise, including crew. - If a family is travelling, every member of the family has to present reservations with the list of all travelling family members 11. In case of visit of fares/conferences - Proof of registration or a copy of a ticket. 4.
