Family register (issued not later than 6 months before the applying date) 8. Notary invitation from the inviting person. It must be issued by Ukrainian notary. MANDATORY requirements for the invitation: Information of the inviting person - name, surname, passport data (series, number, date of issue, issuing authority) and a permanent or temporary residence (if not Ukrainian), address of residence in Ukraine, mobile phone, the commitment of an inviting person to cover all expenses of invited person during his/her stay in Ukraine and his/her departure from Ukraine Information of the invited person - name, surname, information about its date and place of birth, nationality, passport data (series, number, date of issue, issuing authority), address in Lebanon, purpose of the trip (private), the period of the planned visit to Ukraine, the number of entries and the place of his/her residence in Ukraine. 9. Copies of internal Ukrainian passport / residence permit (if not Ukrainian) of the inviting person – all pages with marks and stamps needed! 10. Booked round trip air tickets from Lebanon to one of the countries bordering Ukraine, namely Poland / Slovakia / Hungary / Romania / Moldova + booked round trip train / bus tickets to Ukraine from one of the selected "transit" countries Documents in Arabic must be translated into Ukrainian or English Please note: In case of the visa rejection consular fees are non-refundable.
