No visa documents are needed for a short visit — Ukrainian citizens enter Poland with just a valid passport. If you apply for a longer-stay visa, these are the exact documents required, by visa type, from the official visa application centre.

The main documents required for any visa application for a short visit: - filled with Latin letters (in Polish, English languages or Ukrainian in transliteration) and signed by the applicant himself. Visa application for minor is submitted and signed by one of the parents or legal guardian. According to Art. 16 and Art. 18 of the Supreme Council of Ukraine about passports (Approved regulations on passport and Ukraine on Ukraine passport for travel abroad can be found here (zakon1.rada.gov.ua) ), within 1 month from the date of change of surname, passport must be replaced accordingly. Important: surname, first name and middle name must be identical in both internal and international passports, and in the other accompanying documents as well. - Medical insurance – original and copy , which meets to the following criteria: - insurance coverage should be minimum € 30,000 and valid in all EU countries (insurance covers all expenses that may arise during stay in this territory due to the need to travel back for medical reasons, the need for urgent medical care, emergency hospital treatment or death. - franchise not more than 150 € (0 or 50 €). - €30000 coverage valid in all EC countries with maximum deductible not more than €150. - medical insurance should be typed only. - all applicants’ data in the policy should be correct: Name, Surname, full address, passport’s number and date of birth. - in case of one entrance - medical policy should cover all days of the planned stay, in case of multi entrance - medical policy should cover at least first trip. - medical insurance has to be provided with original and copy of payment receipt containing Name and Surname of an applicant or policy number. Taking care about the protection of the rights and interests of citizens of Ukraine, we draw your attention to the fact that according to the Act No. 265/95-VR as of 06.07.1995, as of 28/12/2014, documents confirming the fact of payment for insurance must be in the form of: transaction banking receipts; confirmation (receipt) from the payment terminal; fiscal receipt; numbered payment receipt. Accepting the fee for insurance services via cash desk should be carried out by means of a cash register. Since, handwritten receipts are not recommended. The payment amount must correspond to the amount specified in the insurance policy. - Only an adult can be Insurer. A written request from the religious community registered in Ukraine: name and details (address and telephone), information about type of registration of the community in Ukraine, stating the purpose, duration and frequency of the trips of the representative, information on the scope of duties of the representative (priest of the Orthodox, Catholic, rabbi, imam, etc.) 11.2. Applicants going to Poland for funeral 11.2.1. Original official document confirming the fact of death 11.2.2. Document confirming the family or other relationship between the applicant and the deceased (e.g. birth or/and marriage certificate) 11.3. For persons who are going to visit cemetery 11.3.1. Original official document confirming the existence and preservation of the grave (valid during 20 years from the date of issue). 11.3.2. Document confirming the family or other relationship between the applicant and the buried (birth or/and marriage certificate). 11.3.3. Original and copy of the document from the Red Cross (valid for 10 years from the issuance date). 11.4. For persons accompanying other family members / travelling with other purpose: 11.4.1. Invitation from the respective company / organization/ institution proving purpose of travel. Invitation from the host Company in Poland*: original and copy of invitation OR sent by fax (with senders contact details on it) OR sent via email (needs to be provided along with a screen shot of the recipients email box), and contain the following information: - Related to person invited: name, surname, date of birth, number of valid international passport of the invited person, nationality, Purpose and period of visit (exact dates, quantity of days and entries), information regarding costs for travelling and accommodation, and personal details of children who will accompany the invited person. - Related to inviting company: date of issue of the invitation, name, surname and signature of a contact person in the company, contact phone number, companies stamp and also registration numbers of the inviting company according to polish law requirements (REGON, NIP, KRS**). It is mandatory to provide bank receipt proving prepayment of the Service Fee during application submission. Please note in case not shown application will not be accepted. * Please note that invitation is valid for 3 months from the issuance date. Written confirmations from Ukrainian side are considered valid for 1 month from the issuance date.While paying Visa Fee/Service Fee at the bank inside the VAC please, provide copy of 1 – 6 (with all the notes) pages of your internal passport or copy of first page of your external passport. This document must contain information about the actual year of study and the semester. - Proof of payment for studies if applicable. (issued by an educational institution). Original and copy of the driver license. 10.2.3. Original and copy of the Company’s State Registration Certificate and a License on conducting international transportations; 10.2.4. Work statement and cover letter from Ukrainian company with confirmation that an invited person is a driver and conducts international cargos. It is mandatory to provide bank receipt proving prepayment of the Service Fee during application submission. Please note in case not shown application will not be accepted. In case of visiting close relatives - wife/husband, children (including adopted), parents/custodians, legally residing in Poland – original and copy of written by hand invitation from the host person. Additionally copy of 1st page of passport should be provided if the hosting person is a citizen of Poland; In case citizen of Ukraine possesses a valid residence permit in Poland, then it is required to provide it’s copy. For citizens of Ukraine who are in Poland on the basis of a national visa D and invite their close relatives, it is required to a copy of the first page of the passport, copy of the valid D and copies of pages from passport with stamps of entry/exit from the territory of Poland.
