Philippines Visa for Emirati Citizens

- Personal appearance of the minor applicant at the ePassport Renewal Centre is a must. Minor must be accompanied by either parent. - Printed manual application form for Dubai & Abu Dhabi duly completed. - Original Passport - Photocopy of current Passport data page and Electronic Visa copy / Emirates ID - Child born in Philippines, Original and copy of the Philippine Statistics Authority (PSA) issued Birth Certificate - Child born Abroad (outside Philippines), Original and copy of the Philippine Statistics Authority (PSA) issued Report of Birth - With full name of the minor as per passport with no single discrepancy - Transcribed Birth Certificate from the Local Civil Register is required when entries in the PSA Birth Certificate are blurred or unreadable. - Copy of any VALID ID (school id, etc). - Photocopy of passport data page and visa of both parents - PSA Marriage Contract (married in the Philippines) or PSA Report of Marriage (married abroad) of parents (issued by the Philippine Statistics Authority PSA), for married parents. - If one of the parents is already deceased, a PSA-issued Death Certificate / PSA- issued Report of Death is mandatory. Note: Non- Philippine Statistics Authority (PSA) documents will not be accepted. Printed Document should not be blurred Important Instruction for Abu Dhabi applicant - Personal appearance of the child and either parent. If the child’s age is seven (7) years and below, only the mother can apply for the child’s passport. - Certificate of Marriage/Report of Marriage issued by the Philippine Statistics Authority (PSA) duly authenticated by the Department of Foreign Affairs (DFA) in Manila is required if only the father files the application. (Original and photocopy) - Certificate of Live Birth/Report of Birth issued by the Philippine Statistics Authority (PSA). (Original and photocopy) - Passport of both parents and valid ID (ex.
