The letter should contain the following information: Personal data of invited people – name, surname, date of birth; Purpose, time and the duration of stay (stays); In which capacity the invited people travel to Lithuania; The Responsibilities of the Receiving Party to guarantee and provide the accommodation. The invitation from Legal entity should be issued by its legal representative who has the signing authority. In case there is an appendix attached to the invitation (for instance, the list with the data of invited people), it should be issued as well as the main document. Invitation can be submitted in original, as a fax or printed scan of the original document. Note! For faxes and scans of the document with original seal and signature should be used. The documents with scanned and pasted signatures can't be submitted. If there is no invitation available for submission, its obligatory to provide another confirmation the purpose of travel (for instance, information about the participation at the conference, entrance ticket, commercial correspondence, business trip program etc.). If the Receiving Party organizes or guarantees accommodation, then the invitation should contain the details about accommodation (name of hotel, hostel, etc. and it’s address), the precise period during which the Receiving Party provides the accommodation. If the invitation presupposes several travels, then it’s allowed to provide the data about accommodation valid only for the 1st travel. The invitation from the Receiving Party should be issued by it´s legal representative who has the signing authority. In case there is an appendix attached to the invitation (for instance, the list with the data of invited people), it should be issued as well as the main document. The Embassy reserves the right to ask for additional documents (for instance, place of residence confirmation, transport documents, etc.). 2. Passport and the copy of the pages in duplicate with personal data - passport (travel document) should be valid minimum 3 month after visa expiry date; - should be issued during past ten years; - should contain minimum 2 blank visa pages aimed. 3. UK Residence permit and Share Code – Original and Copy It is mandatory to bring the printout of the share code from the website gov.uk ↗ (gov.uk ↗) in addition to UK Residence permit, endorsed in the passport or issued as a Biometrics card, must be valid for at least 90 days beyond the return date of the trip to the Schengen Area; C type visitor visa (see here (vfsglobal.com ↗)) is not acceptable. 4. Travel Medical Insurance - Travel Medical Insurance, which shall cover the entire period of the intended stay, be valid for all Schengen States and cover any expenses which might arise in connection with repatriation for medical reasons, urgency medical attention, emergency hospital treatment or death during the stay. The minimum coverage must be 30.000 EUR. If a bank covers you while abroad, a recent letter from the bank stating that the above conditions are fulfilled must be provided. Please be advised that the 'E111-form' or its successor, the European Health Insurance Card (EHIC), is not an adequate travel insurance. If an Applicant requests for multi-entry visa it’s acceptable to submit adequate and valid travel medical insurance covering the period of their first intended visit. 5. Proof of employment - General employment: recent (not older than one month), official and signed original letter from employer (headed letter with name, position of signatory and date of issue, address, recent telephone number and registration number in the United Kingdom) and/or payslips for the last 3 months.Please note contracts of employment are not accepted. - Self-employment. Recent original letter from accountant, banker or solicitor (official headed letter with name, date of issue, address, recent telephone number and position of signatory and registration number in the United Kingdom) stating self-employment or business ownership in the United Kingdom, and letter from the tax authorities. - Students. Recent (not older than one month), official and signed original letter from United Kingdom school, college or university stating date of issue, name of the applicant and type of studies and number of lessons (hours) per week and attendance record. - Unemployed. Social benefit entitlement letter. 6. Bank Statement Last 3 months current account UK bank statement (original and copy) in your name and address showing details and a balance proving that you have sufficient means of subsistence both for the duration of the intended stay and for the return to the United Kingdom or your country of origin, amounting to a minimum of € 40 (approx. GBP 34) per day or - traveller’s cheques showing the same as above. 7. Proof of confirmed and fully paid tickets (To & Return) Proof of confirmed and fully paid tickets to the United Kingdom or to the country of final destination. If travelling by car, ferry tickets must be fully paid and showing all ports of entry and re-entry. Originals and copies of car registration and insurance documents must also be provided. If traveling among multiple Schengen countries, travel tickets are mandatory between every border 8. Minor children: - Copy of birth certificate. - If a minor child leaves the country accompanied by one of the parents / guardian, who already has a valid Schengen visa, the copy of this visa has to be attached to the documents. In this case the consent signed by both parents or guardians and allowing the child to leave the borders of the UK is not needed. - In case a minor child leaves the country and is accompanied by any other person except for parents/ guardians the original and a copy of notarized consent signed by both parents or guardians is attached to the documents. If a child has only one parent or guardian, the confirming documents are provided (for instance, death certificate of another parent, single parent document, etc.). 9. Documents confirming the purpose of travel For patients: - document from the healthcare institution confirming the necessity of medical services; - document confirming place of residence (provided in case if the information about place of residence is absent in the confirmation from healthcare institution). If an Applicant requests for multi-entry visa it’s acceptable to provide the documents on place of residence valid for the 1st entry only. - The confirmation of financial means sufficient for payment for medical services, deposits or prepayment for medical services; For attendants - document from the healthcare institution confirming the necessity of medical services; - document confirming place of residence (provided in case if the information about place of residence is absent in the confirmation from healthcare institution); The confirmation of financial means sufficient for the period of trip. The sufficiency of financial means is confirmed with bank statement where the account activity during last 3 month is reflected (the document can be printed using online banking data and contain website details) or bank reference (within up to 5 days with bank seal affixed) confirming sufficient amount of financial means at the international plastic card "Visa", "Mastercard", "Maestro"and etc.; - in case the Attendant’s documents are submitted separately from the Patient’s documents The Patient’s passport and visa copies are required.
