Minor must be accompanied by either parent. - Accomplished Online Application Form click here (https://onlineappform.passport.gov.ph/) or download the form. - Original Passport - One (1) photocopy, data page of the current Philippine passport. - Original and one (1) photocopy, PSA-issued Birth Certificate of the minor applicant - If the minor is an illegitimate child, personal appearance of the mother is required. - If either parent is unable to appear, the following must be presented: - A Special Power of Attorney (SPA) authorising a representative to assist the child in the application process - A copy of either parent’s valid passport - A copy of the valid passport or photographic ID of the authorized representative - Original, valid Philippine passports of the parent accompanying the minor applicant - One (1) photocopy, data page of the accompanying parent’s valid Philippine passport - Original and one (1) photocopy, PSA-issued Marriage Certificate of parents Important Instruction for applicant: - Personal appearance of the child and either parent. If the child’s age is seven (7) years and below, only the mother can accompany the child during passport application. - If the child is accompanied by his/her father during passport application, the original and one (1) photocopy of the parents’ PSA-issued Marriage Certificate or Report of Marriage must be presented.
