If the mother is outside US, the SPA must be authenticated by the Philippine Embassy/Consulate/DFA where the mother is physically located. - Affidavit of Support and Consent to Travel (ASC). If the mother is outside US, the ASC must be authenticated by the Philippine Embassy/Consulate/DFA where the mother is physically located. - If the person accompanying the minor is a legal guardian/representative, bring the following: - Valid ID* of the guardian/representative - Photocopy of this ID - Photocopy of the mother’s valid ID*.
